As you’ve just finished your first major composition for the quarter, I’d like you to spend some time reflecting on the work you’ve done. As we’ve discuss throughout the HCP research stage, the HCP forms the foundation for your AP. Before we jump into the next assignment, now is a great time to reflect on the skills and insights you’ve gained regarding the composition process so that you can become more aware of them and potentially engage them in your next assignment.

Additionally, this piece of writing can become useful for your ePortfolio Reflexive Introduction – an assignment you will be working on at the end of the quarter. Just like the process assignments for your HCP and AP, starting this type of reflexive writing early, and earnestly, will help you prepare for your final composition of the quarter well ahead of time.

PROMPT

In at least 750 words, engage the following questions. Feel free to format your composition in bullet form, but make sure your responses are in complete sentences and provide explanatory detail.

1. What are some of the strongest/weakest portions of your essay? What makes them strong/weak? Ability to condense and analyze scholarly debates? Your own analysis of evidence? Rhetorical strategies?

2. What part(s) of the essay did you work the hardest on? If this doesn’t correspond to your strongest portion(s), what did you learn about your own writing process while composing and revising this section?

3. Throughout the past weeks you’ve completed numerous “process” assignments in addition to your AP Prospectus and Drafts. Which of these assignments was the most helpful? How was it helpful in your drafting and/or revision process?

4. Given all these process assignments, what is your favorite method of pre-writing (e.g. outlining, concept-mapping, keyword development, source analysis & evaluation, etc.). Why? Is this the same method of pre-writing you discussed in your RJ#1 – Reflexive Self Assessment? If not, how did these assignments help you look at the process of composition differently? How has this changed since researching your HCP?

5. A major component of your HCP is multi-modal composition – not only integrating and analyzing images, infographics, charts, etc., but also thinking about your composition in spatial and visual terms beyond the paragraph/prose form bound by linguistic means of argumentation. How did these new composition techniques help shape and guide your written arguments? How has this changed since researching your HCP?

6. Writing, as you may have learned, is a timely, and deeply personal, process with varying degrees of psychological and pragmatic pressure. How much time did you dedicate to the writing process (hours/week)? How early did you start? Did you find the amount of time you spent on the writing phase sufficient? What barriers did you experience in carving out time for writing? What psychological barriers did you experience while writing (anxiety, distraction, confusion, frustration, etc.)? What did you do to mitigate those feelings? How has this changed since researching your HCP?

7. Did you visit the Writing Center or make an appointment with a research librarian? If so, how was your experience? Describe what you learned and how you applied it in your work.

Scenario

For the purpose of this assignment, assume that you are the manager of the HIM Department of General Hospital. The hospital is due for a Joint Commission accreditation survey in six months. The CEO has asked for a report regarding medical staff compliance with Joint Commission requirements for delinquent records last year.

Information Regarding Joint Commission Standards

The following are Joint Commission standards that address timeliness of medical record completion.

Information Regarding General Hospital’s Policies, Procedures, Rules, and Regulations

To implement this standard, General Hospital’s policies and procedures state:

  • The medical record delinquency rates are monitored on a monthly basis;
  • Action will be taken when a problem is indicated; and
  • Data will be available to demonstrate improvement.

In addition, to ensure the timely entry of all significant clinical information into the patient’s record, the Medical Staff Rules and Regulations list the following documentation requirements:

  • The total number of delinquent records may not exceed 50% of the total number of inpatient discharges and ambulatory surgeries performed for the month.
  • The total number of delinquent history and physicals may not exceed 2% of the total number of inpatient discharges and ambulatory surgeries performed for the month.
  • The total number of delinquent operative reports may not exceed 2% of the total number of inpatient operations and ambulatory surgeries.

Assignment Instructions

Download this Delinquent Record Report and use the data it contains to complete the following:

Part-1: Create Delinquent Record Report Table

  1. Using Microsoft Excel, input the data from the Delinquent Record Report. Include report, column and row titles, and the monthly data.
  2. Use Excel to calculate and automatically insert the total number of discharges for the month.
    1. Total discharges = total inpatient discharges + total ambulatory surgeries.
    2. Total operations = total ambulatory surgeries + total inpatient operations.
  3. Use Excel to calculate and insert the number of delinquent records, history and physical exam reports (H & Ps), and operative reports permitted according the Medical Staff Bylaws for each month in 20XX based on the hospital’s statistics for that month. To calculate the year’s figure, the monthly average should be used. Therefore, have a final monthly average column and use Excel to compute the averages for all rows.

Part-2: Create Line and Bar Graphs

  1. Using Microsoft Excel, construct both line graphs and bar graphs showing the following:
    1. The hospital’s actual delinquent records compared to the total number of delinquent records permitted by the Medical Staff Rules and Regulations for the 12 months in 20XX (do not include the total for the year)
    2. The hospital’s actual delinquent H & Ps compared to the total number of delinquent H & Ps permitted by the Medical Staff Rules and Regulations for the 12 months in 20XX (do not include the total for the year).
    3. The hospital’s actual delinquent ORs compared to the total number of delinquent ORs permitted by the Medical Staff Rules and Regulations for the 12 months in 20XX (do not include the total for the year).

Part-3: Write Narrative Analysis

  1. In a Word document, provide a narrative analysis of the findings for the CEO.
    1. Reference the table and use the graphs you prefer to illustrate compliance in the report.
    2. Include in the analysis whether the hospital was in compliance with the Medical Staff Rules and Regulations in each of the three areas during each month of the year.
    3. Was the hospital in compliance for the whole year?
    4. Be sure your analysis is free of spelling and grammar errors.

Submit the following to your instructor:

  1. Delinquent Record Report Table
  2. Line and bar graphs
  3. Narrative analysis of findings

1) DISCUSSION QUESTION

Part 1: Post a Response

Our focus this week is on ecology. Ecology is the study of interactions among organisms and between organisms and their physical environments. Watch the video entitled “Some animals are more equal than others…” found at: https://www.youtube.com/watch?time_continue=1142&v=hRGg5it5FMI. Then address the following two issues:

  • Why do you think that many keystone species are predators at the top of the food chain in their respective ecosystems?
  • What do you think is the most interesting or significant facet of the video?

2) PEER REVIEW #1 (JESSICA)

Hello Dr. Cox and Class,

Why do you think that many keystone species are predators at the top of the food chain in their respective ecosystems?

Keystone species as these species have the capacity of ensuring the balance of the ecosystem by controlling the number of herbivores found in an ecosystem. These predators play a role in regulating the ecosystem. These predators at the top of the food chain when you remove the predator from ecosystem it affects this ecosystem. The species decrease as one species pushes out other herbivores. This keystone species guarantees healthy completion between the various species therefore ensuring a balance in this ecosystem by limiting the domination of any one species.

What do you think is the most interesting or significant facet of the video?

The most interesting facet of the video is the manner that ecologist Robert Paine actions to remove Starfish from this ecosystem led to the domination of the muscles in this ecosystem. The Starfish exclusion identified the role that keystone species play in ensuring that the ecosystem is diverse by limiting the domination of one species. The Starfish was identified as a marine keystone species through the impact it had on the population sizes of other species. Therefore, the removal of the keystone results in the collapse of the ecosystem.

3) PEER REVIEW #2 (SHERRY)

Hello Professor and Classmates,

Why do you think that many keystone species are predators at the top of the food chain in their respective ecosystems? From my understanding of these keystone species I believe that the keystone species not only assist but guarantees healthy relationships amongst these different species, in other words, ensuring a well-established balance in the ecosystem by limiting the strength and domination of anyone specific species.

What do you think is the most interesting or significant fact of the video? The most interesting fact from the video to me is how ecologist Robert Paine demonstrated by removing the starfish species that resulted in major failure within the ecosystem.
